Spring Registration Is Now Open

Please return Athletic Physical Forms by Thursday, February 22, 2024

 

Please turn in the forms directly to the Health Office. Do not email physical forms.

         Completed Health Forms are to be submitted as follows:

WHS Students / UCVTHS / Magnet ---> WHS Health Office or placed in the bin on the table outside the front doors at WHS marked "Athletic Health Forms"

Middle School - to your school nurse's office

 

If we have a current physical (within one year of the start date) on file - on our forms,

then only the Health History Update Questionnaire is required.

If you are new to the program and have an N.J. physical within one year of the start date,

download the Physical Form Packet and have your physician fill out the forms

otherwise, a new physical will be required to participate.

- A credit card for Student Activity Fee payment 

PLEASE NOTE: If you are doing multiple registrations, you will need to leave 10 minutes between registrations for your card to go through
